Using 1 strand of embroidery floss:

There are two versions on the same chart. 

Use an off-white or natural linen fabric
Version 1: DMC flosses 3831, 3712, 926, 3072, 927 (not supplied).
Version 2: DMC flosses 3688, 3689, 451, 453, 452 (not supplied).
